What does it mean when the primordial Teleporter aligns with the moon?
When the spires are in their default state of a near-circular spiral, the Teleporter will be "aligned to the moon" and will send players to Commencement.
If you reach the teleporter on stage 5, sky meadow, it is aligned to the moon by default. So if you click the teleporter itself, it will port you to the moon after killing the teleporter boss.
To change which stage you go to (either the boss stage or just looping back to the first map), you need to interact with the outside of the teleporter which will then “shift” the alignment of the teleporter. A message will appear in chat saying either “aligned with the moon” or “aligned with the planet”.
From here you can see the message, “The Primordial Teleporter aligns with the moon..” This signals that the Primordial Teleporter is ready to transport you to the new lunar environment, Commencement.
Teleporter charge time scales linearly with the number of players standing inside the red dome, out of the total number of players currently alive. This requires a minimum of 90 seconds if all players alive always stay within the red dome.

Looping in Risk of Rain 2 involves completing sets of five stages. Each time you complete the fifth stage Sky Meadow, the game will loop back to the first environment. Item cost and difficulty will increase with each loop, and new monsters will begin to appear.
A Blue Portal is a portal that teleports the players to the Bazaar Between Time. To have one spawn, a Newt Altar may be activated, or a Blue Orb must randomly spawn. This Blue Orb can be seen circling the Teleporter if this is the case. After the teleporter event charges up to 100%, the Blue Portal will spawn.

Find a Newt Altar somewhere in Sky Meadow and interact with it to make a blue orb appear, which will allow you to enter the Lunar Shop via blue portal after the teleporter event. You get to skip Mithrix, loop, and go shopping if you go this route! Perhaps the more appealing option of the four.
As with the first game, you'll need to beat the final boss at least once to be able to easily loop to your heart's content in later runs. However, it is possible to loop without beating Mithrix at all.
So, the reason why many players cannot find the teleporter at risk of rain 2 isn't that it doesn't exist. It's because the teleporter has spawned in an area inaccessible to you. Since it's not possible to reach that area nor see it, you can end up searching for a house and still come up empty-handed.
Once you're outside of the sphere, destroy the boss, making sure to leave a few enemies remaining. Now, as soon as you enter the sphere again, the teleporter will immediately reach full charge. Before you do that, allow enemies to attack you until you're under 10% health.
The Obelisk is a structure found in the Hidden Realm: A Moment, Fractured, which can only be reached via the Celestial Portal, which spawns every three levels after the first loop next to the Teleporter.
You must shift the Primordial Teleporter's destination before the teleporter event. Once you've shifted the teleporter's alignment, you can use it to be whisked back to the start, either to the Distant Roost or Titanic Plains, and avoid Mithrix and his terribly bleak moon.
To reach the Moon, players must get to Sky Meadow and locate the Primordial Teleporter, which looks very much like a standard RoR2 teleporter surrounded by silver prongs. Once the Primordial Teleporter has been found, fans should interact with it to spawn a standard boss that they must defeat before going through.
The Focused Convergence is a lunar item in Risk of Rain 2. It makes Teleporters charge 30% (+30% per stack) faster, but decreases the charging radius by 50% (-50% per stack).
